Design a data capture sheet for the family to fill in.

Remember, people who fill in forms often make mistakes so make sure yours is as easy to fill in as possible. It is important that the form is as accurate as you can make it as it will be used as the outline for your database.

Think carefully about the format of the form and the questions you want the family to answer. You should also consider the field types you will use in the database to make the data entry easy and to avoid as many input errors as possible.

Resource Sheet 7 will help you with the design of both the data capture sheet and the database.

You will need to evaluate the sheet to make sure it is as accurate and as easy to fill in as possible. It will also ensure you have all the information you need for the task.

Produce an evaluation form.  Ask THREE people to fill in both the data capture sheet and the evaluation form and tell you what they think of the sheet.  Ask if they could suggest any improvements that you could make.
